The first statement starts with an exclamation—and exclamations are another way of showing intensity.

In this case, while it shows the narrator is convinced that what he is saying is true, it’s so extreme and shouty that we wonder if it’s false.

Let’s add a few exclamations to our simplified snippet.

Of course! Nervous — very, very, dreadfully nervous I had been and am, but why will you say that I am mad?

Fine! Sad — so, so, deeply sad I had been and am, but what makes you think that I am suicidal?

Yes! Angry — very, very, violently angry I had been and am, but I am not murderous.
